APPLICANT INSTRUCTIONS FOR PLACEMENT ON THE NEW YORK AREA QUALIFICATION LIST

DGACA-EAST REQUIRES ALL APPLICANTS TO CALL THE OFFICE AT 914.747.2979 TO DISCUSS THEIR APPLICATION PRIOR TO SUBMISSION.

Every applicant must submit an Application Form and Production Header/Summary Sheet, as well as the supporting documentation listed below. An individual submitting an initial application is also asked to submit an Additional Data Form indicating his or her sex and ethnicity. The Additional Data Form is optional and the information is for administrative use only.

Applicants are expected to submit documents in 3-ring binders (preferably not larger than 3 inches) by production separating each with tabbed dividers.  For episodic television each season is to be organized as a separate production.

Within each production organize the production reports, call sheets, pay documents within separate batches in chronological order (day 1, 2, 3 etc.). If documentation is not organized, it will not be reviewed.

Highlight your name on each document submitted (front and back).

Check that photocopies are legible; they should not be too light.

DO NOT reduce legal size documents to letter size. When placing sheets in the binder do not fold, they may extend beyond the top or bottom of the binder.

Documentation required for each project:

  • Deal Memo or Start Slip
  • Pay documents substantiating compensation for every workday claimed
  • Production Reports* (front and back) for each workday claimed with the shooting company
  • Call Sheets* (front and back) for each workday claimed with the shooting company
  • Crew Lists
* Stage Managers/Associate Directors: If Production Reports and Call Sheets are not available, please submit rehearsal, blocking and taping schedules. If none of these is available, and you were employed by a station, submit weekly schedules or Associate Director/Stage Manager Assignment Sheets along with a letter from the Station Executive verifying your job title, duties and dates of employment.


Pursuant to Section 14-901 of the Alliance of Motion Picture & Television Producers and the Directors Guild of American , Inc. Basic Agreement of 2014 “Any person claiming to have fulfilled the Qualification Lists requirements herein must make a written application… no later than six (6) months following the completion of the last work assignment to be considered. Such person shall have the burden of establishing and proving any such claim by appropriate documentation.”

To receive credit for days worked, the applicant must prove he or she was compensated. In most cases, pay stubs are sufficient evidence of payment. When pay stubs are not available, the DGACA-East will consider copies of cancelled checks, earning summaries, W-2 or 1099 forms, earnings summaries from DGA Pension, Health & Welfare and printouts from the payroll company. Invoices and letters from production companies are not accepted as proof of payment.

It is the applicant's responsibility to demonstrate to the DGACA-East's satisfaction that he or she has met the requirements for placement. The DGACA-East reserves the right to request original documents if they are needed to verify the application claim; however, applicants are urged to submit photocopies of their documents. The DGACA-East assumes no responsibility for the loss and/or damage to any documents (original or duplicate) that are submitted.
